Students who are enrolled in the 5th – 8th grades and are planning on participating in school sports are required to have a sports physical form completed each school year by their physician. This form must be on file in the school office prior to any student participating in any Parochial Athletic League game or practice. Participation in after-school sports is contingent upon: availability of qualified volunteer personnel; try-outs (if deemed necessary by the coach); payment of the current athletic fee to help defray the costs PBVM pays for PAL fees, equipment and uniforms; parental permission form; and a student’s academic and behavioral standing with the school.
